Policies
Arrive 15 minutes early for appointments (20 minutes for new patients).
Please call at least 24 hours ahead to cancel or reschedule.
Co-payments are due at the visit. We accept cash, checks, and major credit cards.
Bring your current insurance card each time and let us know of coverage changes.
